Form Completion Policy
Good Stress Company, PLLC does not provide disability letters, companion pet letters, letters regarding your ability to work, or any letters that would inform providers of your mental health history. You must discuss these needs with your medical or primary care provider.
Services provided outside of routine medication management or psychotherapy visits may incur additional fees. These fees are based on the time required by the clinician to complete your request. Please note that your insurance covers only your direct medical and therapeutic care. It does not cover activities such as communication with school personnel, drafting letters, or completing medical leave or disability claim forms.
At the discretion of your provider, you may be billed for these services, and you will be notified of any associated charges in advance. You reserve the right to withdraw your request if you do not wish to incur these fees. Please note that medical leave and disability claim forms are only completed for established, long-term clients, who have typically attended more than eight (8) sessions. Additionally, a mental health diagnosis does not automatically equate to disability or functional impairment. For example, individuals with major depressive disorder are not considered legally disabled if they are capable of “substantial gainful activity,” as defined by the Social Security Administration.
Fees & Processing
Psychiatry- $75 per page (paid in advance) for form completion or letter drafting.
Psychotherapy- $45 per page (paid in advance) for form completion or letter drafting.
Forms typically require 5–7 business days for processing.
A $25 rush fee applies if you request completion sooner.
Forms must be fully completed by the client or Legal Guardian before submission, including all relevant medical history and medications from other providers.
Completed forms must include clear submission instructions and, if mailing is required, a self-addressed stamped envelope.
Providers will not falsify dates or information under any circumstances.

Because form and letter preparation is administrative in nature and not considered direct patient care, it is not reimbursable by insurance. The expense is therefore the responsibility of the client or their responsible party. Time for completion may range from 15 to 90 minutes, depending on the complexity of the request.
Cancelation/No Show
24-hour notice is required for cancellations – Failure to cancel within the 24- hour notice will result in a late cancellation fee of $50.
No- Shows will result in a $50 fee.
Co-Pay
Co-pays are due at time of service, since we are currently seeing patients virtually all co-pays will be charged the day following the session. There is a $35 fee for return checks.
